Community Health Construction
For more than a decade, the CMUH has been deeply involved in the communities to develop the community health care, and has established solid community network foundation. In terms of community health construction, it has integrated 42 community group resources and set up 6 community health stations to promote the integrated health screening, to develop community health promotions and to practice the preventions and treatments of cancer and chronic diseases. It also offered regular the weight loss classes, walking classes, smoking cessation classes, health classes and other health promotion courses and activities for at-risk community groups and cases of illness. It passed the community health promotion certification of Bureau of Health Promotion in 2009, was awarded the excellent unit of community health construction assessed by the Bureau of Health Promotion in 2011, and won many Hospital Health Promoting Competitions conducted by Public Health Bureau, Taichung City Government, such as the outstanding aging service award, the outstanding smoking cessation service award, the excellent weight loss service award, the creative construction presentation award, etc.
The CMUH integrates the APP on smart phones developed by CMUH and the health care website of CMUH and Asia University to provide real-time community construction activities and health education information for people. The APP won the Healthcare Quality Improvement Campaign – Smart Medicine issued by Joint Commission of Taiwan in 2015 and the Symbol of National Quality (SNQ) of Institute for Biotechnology and Medicine Industry (IBMI) in 2016.
Medical Resource Integration Care IDS
The CMUH has cooperated with 200 medical practitioners in 167 clinics to promote the shared cares of community medical group with family physicians system. The community medical group is the largest in Taiwan and monitors the health status of 150,000 community residents.
The CMUH develops the integrated medical care model focusing the cases in a family within the scope of community. Through the integrated information communication platform between the CMUH and clinics, the Preventive Medicine Center sets the 24-hour Community Care Center of the community medical group to provide 24-hour continuous, all-dimensional and collaborated health management services for the members, including the preventive health services for community diseases and cancer screening in early detections and treatments, the home care services in which family physicians participate, the remote care services for stroke cases, etc.
According to the auditing results of family physicians’ integrated health care quality enhancement announced by the Central Division of National Health Insurance Administration in 2007 and 2008, the scores of the community medical groups cooperating with the CMUH in the quality indexes and satisfaction are all higher than the average score of the other groups in the central Taiwan. It raises the overall health care quality and reduces the medical resource consumptions by saving about 100 million NTD expenses in modern medicine clinics every year for National Health Insurance Administration in average. The complete Integrated Health Care Service Model not only improves the overall medical care quality, but also eliminates the medical resource wastes efficiently. It won the Symbol of National Quality (SNQ) of Institute for Biotechnology and Medicine Industry (IBMI) since 2009 to 2011.
Mountain Medicine Integration Care IDS
Based on the plans and instructions of Provincial Health Department since 1998, the CMUH cooperated with Nantou Hospital of Health Administration to provide 24-hour resident medical services for Dili and other 3 villages in Sinyi Township. Furthermore, to implement the mountain medical health cares, it was responsible to organize the medical cares since July 15, 2000. Because of the outstanding efficiency, it was responsible for the medical payment benefit improvement program for Sinyi Township since 2001 to 2010 to actively solve the health problems of people in the Township. It won the Symbol of National Quality (SNQ) of Institute for Biotechnology and Medicine Industry (IBMI) in 2002.
